Ontario Brain Injury Association Research Questionnaire

-Child/Youth (under 16 years of age)-

Thank you for taking the time to complete this survey. It is designed to provide accurate information about the long term outcomes for people who are living with the effects of acquired brain injury (ABI). The information will be held in strictest confidence, and be used to provide statistical reports that will be useful to health care policy makers, insurers and researchers who are investigating ways that people with ABI can be better served. You can view and download the report at www.obia.on.ca or you can request a hard copy of the survey. At no time, will any personal data be revealed. From time to time, we are approached by researchers who are seeking information other than that in the survey. When that happens, our office contacts the appropriate people who have responded to the survey and offers them the opportunity to participate in the research project.

The information that we gather on this survey can only come from people like you. You are the experts about life with brain injury. It is our hope that the information that you provide will contribute to improving the lives of people who have experienced brain injury. In appreciation for your effort in filling out this survey we are offering you a one year free dual membership in both OBIA and the local brain injury association of your choice. Your membership will give OBIA a greater voice to speak on behalf of people living with brain injury in Ontario.

  • A biennial provincial conference for both professionals and those living with the effects of brain injury
